The default shipped path has plan, diff and phase-diff reviews off, so strong verification comes only after code and pre-code and interaction review are not the normal default.

At HEAD `f354864` (v3.3.0)

Factually correct. cadence-core/route-table.json shipped row - plan: off, diff: off, risk_surface: blocking, phase_diff: off - and cadence-core/templates/config.json:3 ships "stakes": "shipped".

Note

This is a deliberate cost decision, revisited and re-set as recently as the 2026-08-12 defaults pass, and it is configurable per project. Run cost is a stated project priority.

The consequence worth acting on is that the one remaining gate has no proof it ran - filed and open separately.
